The Health and Care Worker visa itself asks for the same English as the Skilled Worker route: CEFR B1, which is band 4.0 in each skill on IELTS for UKVI. That is not the bar that matters. Every clinical role needs professional registration before the sponsor can issue a certificate, and the regulators set their own, much higher scores: 7.0 overall for nurses and midwives (6.5 in Writing), 7.5 overall for doctors, and 7.0 overall for most allied health professions.

- Meet the regulator and the visa's own English requirement is met by the same certificate, as long as it is the Academic test from an approved centre.
- The regulators accept the OET as an alternative (grade B for nurses and doctors, C+ in Writing for nurses), which some candidates find easier than IELTS Academic reading.
- Care workers and senior care workers without a registered profession only need the B1 visa bar, but that route's sponsorship rules have tightened; check the current Home Office guidance before relying on it.
- Recruiting trusts commonly pay the visa fee and health surcharge once registration is done, so the English score is usually the gate on the whole move.

Is IELTS 4.0 really enough for a nursing job in the UK?
For the visa, yes. For the job, no: the NMC will not register a nurse without 7.0 overall (6.5 accepted in Writing) on IELTS Academic or the OET equivalent, and no registration means no certificate of sponsorship.
Can I combine two sittings for the NMC?
Yes. Two Academic sittings within six months can be combined if no skill drops below 6.5 in either and the combined result meets the full requirement. The GMC does not allow combining.
